ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: Ad-I9Rn-add <br />INSPECTION DATE: ~-Find INSPECTOROS INITIALS: ~~ <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site on 2/13/04. <br />Present during the inspection were Tim Comer and Phil Barnes of CC&V. The <br />main purpose of the inspection was to make sure that the Phase II high <br />Volume solution collection system was at or below the 47-foot level, which <br />is the 80~ operational maximum volume allowed under amendment # 8. On <br />Wednesday, 2/11/04, Mr. Barnes as required under Board Approved Amendment # <br />8, notified the Division that the volume in Phase II PSSA area had reached <br />47.2 feet and that CC&V, had taken the steps to reduce the volume to the <br />approved 47 feet. <br />The following readings were recorded, from the Phase II HIGH AND LOW VOLUME <br />SOLUTION COLLECTION SYSTEMS. <br />High Volume solution collection transducer # 4 was recorded at 43.5 #5 was <br />at 46.6 and # 6 was at 45.6. Low volume solution collection transducer # 1 <br />was recorded at .23 and # 2 was at .63. Piezometer for the preg. Solution <br />was recorded at 46.3 feet. (All other readings are in feet of hydrostatic <br />head) <br />Inspected the Phase I AND IV, pregnant solution collection areas and <br />recorded the following readings from the High and Low Volume solution <br />collection transducers. <br />Phase I high Volume solution collection transducer # 1 was recorded at 48.9 <br /># 2 was recorded at 51.8 and # 3 was recorded at 52.4.Low volume transducer <br /># 1 was recorded at .21 # 2 was at .38.
