(Page 2) <br />MINE ID# OR PROSPECTING ID# M-1998-054 INSPECTION DATE 6/04/04 INSPECTOR'S INITIALS GRS <br />This was a Pre-operational inspection of the area to be added to the current 110c East <br />Douglas Creek Pit permit to convert it to a 112c Permit, conducted by Gregg Squire of the <br />Division of Minerals and Geology. Stan Rasmussen, Rangely District Supervisor for the Rio <br />Blanco County Road and Bridge Department was also present. The pit is located several <br />miles southeast off of Douglas Creek up the East Fork of the Creek. The area to be added to <br />the permit was examined, now disturbance has taken place on this proposed permit area. A <br />public notice of the proposed conversion was posted at the site. There was no activity <br />taking place at the permitted portion of the site although there were some signs of recent <br />material removal. The operator ie in the process of installing some storm water control <br />measures, including a toe berm below the planned and active disturbed areas. <br />The permit sign and permit markers are in place, no problems were noted during the <br />inspection.
