It is our understanding from Mr. Gary <br />Curtis' exit interview that shortly after pushing of material began, two laborers appeared from <br />the top of the hill and that within ten minutes of starting the push, a QC/QC representative was <br />on the site as well as additional laborers. <br />In order to ensure that a gap in coverage does not reoccur, CC&V has directed Bateman, who <br />in turn has directed Ames, to not start any pushing of drain cover material until clearance is <br />expressly given by Golder Associates based on the presence of adequate staff and otherwise <br />appropriate conditions. Golder has been given unquestioned authority to stop any pushing of <br />drain cover material if conditions are inappropriate. But, equally important is the fact that <br />pushing is not to start until Golder has agreed in advance. In this manner we will control the <br />eager startup by dozers. <br />The material that was pushed without adequate presence of QA/QC and rock-picking personnel <br />was material that had been dumped earlier while rock-pickers and OA/OC personnel were <br />available. Further, as soon as adequate staff was available (within 2 and 10 minutes), all <br />oversize material was removed. Thus we believe that we mitigated the omission in a very short <br />time and that we have taken the appropriate steps to avoid reoccurrence. <br />If our actions have not been appropriate or adequate, please let me know immediately. <br />
