r <br /> <br />MINE ID # OR PROSPECTING ID #M-80-244 <br />INSPECTION DATE 11-4-99 <br />(Page 2) <br /> <br />INSPECTOR'S INITIALS BK_ <br />The Division conducted a final inspection of the expansion areas of the leach pad Phase II <br />AND III for which the operator has submitted the as built certification report. On November <br />2, 1999 the Division received from the operator the final as Built Certification Report for <br />Phase II AND III expansion areas. Present during the inspection was, Mr. Scott Lewis of CC&V. <br />Prior to inspecting the expansion areas, the Division conducted an inspection of the area <br />located south of the load out bin that was clay lined during the last inspection. The area <br />now is covered with two feet of drain cover and no major problem noted. Continued the <br />inspection to the Phase II and III expansion areas for which the operator has submitted the <br />as built certification report. The entire area was inspected. Drain cover was in place over <br />the leached pad area. As part of the Amendment #7 approval which allowed the operator to <br />place ore and leached ore in the area prior to certification approval, the operator has <br />commenced placement of ore and leached ore in the expansion areas. No major problem noted. <br />The cement drop structure for the upland diversion was in place. The original approval called <br />for two cement drop structure, However, the operator submitted a revision to the original <br />approved plan and obtained an approval for only one drop structure. Inspected Leak detection <br />sump# 8 the new design which is totally lined and isolated from major influences of surface <br />water was in place. The new sump design if nothing else might limit major influences from <br />surface water inflow. <br />Inspected the underground workings in the phase three area that is not covered under this <br />certification report. Work was continuing in the area. It is anticipated all construction <br />activities in the area will cease by the end of November. Inspected the area where the <br />temporary Diversion will be placed. The approval for the diversion was granted on 11-7- <br />99.Inspected the main pit area no problem noted. Main pit was not active at the time of the <br />inspection. Inspected the topsoil stockpile area south of the crusher area, it was stable. <br />Inspected the North Cresson area of the Phase III expansion, active mining was taking place <br />at the time of the inspection.
