A culvert was installed in the access road to the pond <br />but there doesn't appear to be any other changes to the road system <br />on the bottom. Roads up to the benches from the bottom area were <br />recently upgraded by shaping and graveling them. <br />Mine records were available for review at the minesite. Their mine <br />permit, C-81-053, expires September 1, 1992 was transferred to L 6 L <br />Energy March 1, 1991. The NPDES permit, CO-0041394 is effective <br />until Junne 30, 1991. Water monitoring records for the mine are <br />restricted to the discharge point from the sediment pond which were <br />up-to-date and being sent in on a quarterly basis as required. There <br />were no recorded flows from the pond on record. The county use <br />permit has been applied for but approval is pending. Self <br />inspections of the pond for maintenance were done according to <br />requirement with the last recorded date noted as April 1, 1991. <br />The as built certification for the pond is mute at this point due to <br />the related enforcement action so I did not require that in the <br />inspection review. <br />The certificate of public liability insurance, policy ll CKR12358, <br />issued by Commercial Union Insurance was effective through June 23rd <br />of this year and included the proper coverage for aggregate and <br />single occurrence. There is no blasting coverage but at this time, it <br />is not being used. <br />The mine has been under temporary cessation since August of 1990 but <br />I failed to verify that a letter was sent to the Division regarding <br />that status.
