(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~-~nm_199 <br />INSPECTION DATE: s~ar~h ~z, vnnn INSPECTOR=S INITIALS: 1111~L <br />OBSERVATIONS <br />This was apre-operational inspection conducted by Tony Waldron of DMG along with Joe Gagliano <br />representing the operator. The site is located approximately 15 miles south of Westcliffe, Colorado and is <br />adjacent to Hwy 69 and County Road 105. The proposed permit area is located on an alluvial terrace that is <br />the target of the mining operation. The required notice was posted in a location that is visible to the public. <br />The permit area is bounded by HWY 69 on the east, Froze Creek on the north and Moss Creek on the west. A <br />little used county road (CR 105) bisects the deposit approximately 500 feet from the north permit boundary. The <br />present use of the area is for grazing and the site is typical short-grass rangeland found throughout this area. <br />The permit area is situated such that the only structure of concern is CR 105. Froze Creek is more than 200 feet <br />away from the permit area and will not be impacted by this operation. Moss Creek is appears to be an <br />ephemeral creek at best and does not appear to carry water in most years. It is a very broad flat drainage (1 /4 <br />to Y2 mile across) of which portions are included in proposed affected area. These inclusive areas will not be <br />mined but may be used for screening, stockpiling, and other ancillary mining activities. With respect the county <br />road, the operator will obtain a written damage agreement with the county or remain more than 200 feet from <br />the structure. <br />This concluded the inspection. <br />I & E Contact Address <br />NAME: Jerald A.
