_~ <br />;~ <br />(Page 2) <br />NINB ID ~ OR PROSPECTING ID ~ N-s0-244 <br />INSPECTION DATE 11-30-94 INSPECTOR'S INITIALS ACS <br />OBSERVATIONS <br />A monitoring inspection of the Phase 1 leach pad construction was conducted. <br />Excavation for the diversion channel - Highway 67 culvert installation was inspected. The <br />diversion channel excavation has eaten into an existing topsoil stockpile below the highway. <br />The topsoil has been moved away from the diversion ditch ,and piled next to the existing <br />vegetated topsoil stockpile. The operator must further assure that the topsoil pile ie <br />protected from erosion or contamination caused by diversion ditch installation or operation. <br />In other words, topsoil stockpile ie not an appropriate diversion channel aide elope. <br />During an inspection of the pad area, it was noted that there was two large holes in the <br />upper VLDPE in the pregnant pond just north of the pregnant solution sump. These holes had <br />been marked for repair. It was observed that the geosynthetice contractor (Gundle) was <br />installing sacrificial VLDPE panels below and onto the "winterization" berm. <br />Drain cover placement onto the Phase 1 toe berm was inspected. The approved drawings show <br />a rim of VLDPE liner exposed at the crest of the toe berm (DWG. NO. 16 "PREGNANT SOLUTION <br />STORAGE AREA DETAILS", details H & G). The earthworks contractor (Ames) indicated their <br />intention to install the drain cover in accordance with the drawings. The Division finds the <br />installation depicted in the drawing to be inappropriate, in that it would leave VLDPE <br />exposed to ultraviolet light. The drain cover should be installed, at all pad margins, as <br />depicted in DWG. NO. 15 "ORE STORAGE AREA AND LEAK DETECTION DETAILS", detail C, with three <br />feet of drain cover over the VLDPE at the pad edge. This will not only protect the VLDPE <br />from the sun, but will also protect underlying soil liner from freeze-thaw cycling. <br />The current upstreaco end of the underdrain was inspected. The flanged end of the 24" solid <br />pipe had been capped. An agreement was previously reached between the operator and the <br />Division that this pipe would be flushed with clean water prior to capping, and apparently <br />this still needs to be done, <br />The Economic Hill waste rock piles were inspected. The operator has proposed using this rock <br />as underdrain fill, and has completed geochemical testing on samples of the rock. The <br />Economic !fill waste rock is primarily granitic. The granitic rock would make an acceptable <br />underdrain fill if it is shown to be durable. However, there ie some volcanic rock mixed in <br />with the piles. If the Economic Hill waste rock ie to be used as underdrain, the volcanic <br />rock must be sorted out and removed. <br />The diversion ditches were inspected. An agreement was reached regarding how run-off was to <br />be handled from a small area (approximately 28 acres) outside the permit boundary to the <br />south. This run-off will be routed along the process plant road to the sump above the Phase <br />1 leach pad.
