MINE ID # OR PROSPECTING ID #: M-i 48n-~aa <br />INSPECTION DATE: Q-ns-ro INSPECTORS INITIALS: aK <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of the site on 9-OS-02. Present <br />during the inspection were Phil Barnes and Mark Tidquist of CCS:V. <br />Inspected the external pond, it was still holding some water from the August rain. While in the area, <br />recorded the following readings from the low volume solution collection transducers. Transducer # 1 was <br />recorded at 13.5 and # 2 were at 10.9 inches of hydrostatic head. No problem noted. <br />Continued the inspection the Phase II pregnant solution collection area and recorded the following readings <br />from the High and Low volume solution collection transducers. High volume solution collection transducer <br /># 4 was recorded at 29.9 #5 was at 25.8 and # 6 was at 30.8. Low volume transducer # 1 was at .10 and # <br />2 were at .19 all readings are in feet of hydrostatic head. <br />Inspected the underdrain discharge area. Work on the permanent cement structure for the pump back <br />system was still on going. It should be completed in the next few weeks. Both the 4-inch pipe and South <br />under drain were discharging. <br />All three ponds below the discharge were holding water, no major problem noted. <br />Inspected the recently completed Phase IV (A) LEACH PAD AREA. The as built certification report was <br />submitted to the Division on 8-30-02. The portion of the pad that was already loaded was being leached at <br />the time of the inspection. No problem noted. <br />Inspected the Phase IV PSSA area, underground remediation work has been completed, no problem noted. <br />Inspected the Phase IV b and C areas. Work back fill and grading work was continuing. Along the inter face <br />of tfie Phase III and IV areas, as approved by the Division to extend the liner and load it with processed <br />ore, the portion along the west side has been geomembarne lined, in anticipation of processed ore loading. <br />No application of solution will take place in these areas, until the PSSA for the phase IV is completed, and <br />certification report is submitted. <br />Leak detection work for the Phase IV (A) (B) areas were continuing, no major problem noted. <br />Inspected the new crusher, it was not active at the time of the inspection. Walked inside the structure and <br />talked to the operators. According to the operators, the production from the new crusher and the size <br />gradation are much better tfian the old one .The old load out bin had already bin removed and the primary <br />and secondary crushers should be removed as soon as possible.
