The problems should be corrected by the <br />dates given, or they will become possible Violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />A D <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />Failure to salvage and protect topsoil as required 07/31/96 <br />under section 34-32.5-~116(g). Specifically, windrowed PB or PV: PB <br />topsoil on top of the pit highwall was loot when a TOPIC(S): TS <br />portion of the highwall unexpectedly collapsed during a <br />blast and, topsoil which had been used in construction <br />of a sediment basin wa.s not being properly protected <br />and had become contaminated with limestone fines and <br />rocks. <br />Required corrective actions are to move the topsoil <br />used in the basin embankment construction and place it <br />in a stable area where future contamination will not <br />occur. This pile should then be seeded with a <br />temporary grass seed mix to protect it from erosion. <br />The embankment should then be reconstructed with other <br />non-topsoil material. Once this is done, a photograph <br />should be submitted to the Division which clearly shows <br />that these activities have been completed. This <br />photograph must be received by the Division prior to <br />the expiration of the corrective action date. <br />The second part of the corrective action is to develop <br />a better plan for salvage and storage of topsoil in <br />advance of mining in the pit area. This plan should <br />detail how the topsoil will be removed, stored, <br />protected, and reapplied.
