(Page 3)
NINE ID f OR PROSPECTING ID ~` M-SZ-141
INSPECTION DATE June 12, 1996 INSPECTOR'S INITIALS AJW CBM
The following problems (PB) and/or Possible violations (PV) (and suggested corrective
actions) were identified during this inspection. The problems should be corrected by the
dates given, or they will become possible Violations. The possible violations should be
corrected by the dates given to reduce their severity when considered by the Mined Land
Reclamation Board. The inspector noted on the previous page should be notified of all
corrective actions taken.
PROBLEMS/POSSIBLE VIOLATIONS
A D
CORRECTIVE ACTIONS
CORRECTION
DATE
Failure to salvage and protect topsoil as required 07/31/96
under section 34-32.5-~116(g). Specifically, windrowed PB or PV: PB
topsoil on top of the pit highwall was loot when a TOPIC(S): TS
portion of the highwall unexpectedly collapsed during a
blast and, topsoil which had been used in construction
of a sediment basin wa.s not being properly protected
and had become contaminated with limestone fines and
rocks.
Required corrective actions are to move the topsoil
used in the basin embankment construction and place it
in a stable area where future contamination will not
occur. This pile should then be seeded with a
temporary grass seed mix to protect it from erosion.
The embankment should then be reconstructed with other
non-topsoil material. Once this is done, a photograph
should be submitted to the Division which clearly shows
that these activities have been completed. This
photograph must be received by the Division prior to
the expiration of the corrective action date.
The second part of the corrective action is to develop
a better plan for salvage and storage of topsoil in
advance of mining in the pit area. This plan should
detail how the topsoil will be removed, stored,
protected, and reapplied. This can be accomplished by
modifying the onsite plan and submitting a letter to
the Division that details these modifications.
PB or PV
TOPIC(S)
PB or PV
TOPIC{S)
