• (Page 2) • <br />MINE ID # OR PROSPECTING ID # M-1991-104 <br />INSPECTION DATE 1/24/01 INSPECTOR'S INITIALS RCO <br />OBSERVATIONS <br />This was a partial inspection performed by the Division to verify several items related to amendment application AM-02 <br />for this permit. The operator was contacted to arrange a meeting to review the written adequacy responses. The meeting <br />was held at the operator's office to discuss the response materials. After the meeting, this inspector proceeded alone to <br />the site for the inspection. <br />The Notice that is required to be posted was observed at the permit area entrance gate. <br />There was a small question about the powerpoles and overhead powerline on the north end of the permit area. <br />Observations made during this inspection revealed that the three poles are in fact within 200 feet of the permit area (at <br />the existing north boundary, which is not modified by this amendmentl~ The poles have a single overhead line currently <br />mounted on them, but the line is apparently disabled. It formerly supplied a sawmill, which has long since been removed. <br />The entire structure belongs to the property owner, and none of it belongs to a utility company. The anticipated effects <br />to the poles would be none, but due to the somewhat low height of the line, a large dump truck with its bed up could <br />possibly contact the line. <br />The other structures in the vicinity of the proposed expansion are the landowner's fencelines. The operator and landowner <br />have submitted a damage agreement, which includes responsibility for damage or changes to the fences. The fenceline <br />locations will be modified after mining and reclaiming. <br />The site appears stable at this time: side slopes have been graded and no water impoundment is evident. A large crushing <br />job was apparently performed later in the fall of 2000, since there are two new stockpiles of aggregate on the pad in the <br />extreme north end. The operator continues to encounter oversize material, which is not readily usable or easily processed <br />as road material. It is used for riprap occasionally, but it is not exported as fast as it is excavated. The operator has <br />inquired about private markets for this material, and/or onsite burial during reclamation earthwork. Either of these options <br />appear viable at this time. <br />There were no problems noted at this time.
