(Page 4) MINE ID # OR PROSPECTING ID # M-1979-034 INSPHCTION DATE 07/29/03 INSPECTOR'S INITIALS ESC The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified during this inspection. The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all corrective actions taken. PROHLSEtB/BOSSIBLS VIOLATIONS ARD CORRECTIVB ACTIONS DATE 1. The reclaimed area west of the pond has been reseeded by the landowner and consists of Yellow Sweetclover and a type of PB or PV:BB Mustard. The landowner intends to reseed this area and the TOPIC(S):RV remainder o£ the site with a mixture o£ Birds Foot Trefoil and 10/31/2003 Alfalfa. However, the approved seed mixture in the permit requires Smooth Brome, Pubescent Wheatgrass and Streambank Wheatgrasa (ae per AM-04). CORRECTIVE ACTION: The operator will need to revise the seed mixture for all or a portion of the site, or reseed the area with the approved seed mixture. A revision to the permit must be filed by Oetobar 31, 2003, or the operator will need to notify the Division as to when they-will reseed the area with the approved mixture. 2. The poatmining land use for the site is rangeland, but the PB or PV:PB landowner is using the reclaimed area for recreation in the form TOPIC (S):RS of a commercial ski shooting range. Such activity is a conflict October 31, 2003 of the approved poatmining land use of rangeland, and will need to be reflected in the permit. CORRECTIVE ACTION: The operator will need to submit a revision to the permit changigg the end land use in this azea to recreation. The ski shooting facilities and structures will need to be depicted in the permit, and the operator will need to submit local county approval for this end land use. A revision to the permit must be filed by October 31, 2003. 3. ,The Division was unable to locate a mine identification sign. PB or PV:PB Rule 3.1.12 of the Construction Materials Rules and Regulations TOPIC(S):8M require the operator to poet a sign with the name of the October 31, 2003 operator, a statement that a reclamation permit for the operation has been issued by the Mined Land Reclamation Boar3 (NLRB) and the permit number. CORRECTIVE ACTION: A mine identification sign will need to be posted at the entrance to the site. The operator must submit proof of compliance (in the form of photographs) to the Division by October 31, 2003.
