i , • (Page 21 • <br />MINE ID # OR PROSPECTING ID # M-82-033 <br />INSPECTION DATE 04/22/97 INSPECTOR'S INITIALS RCO <br />OBSERVATIONS <br />This was apre-operation inspection performed by the Division, in response to a conversion application submitted by the operator, <br />to convert the existing 110 permit to a 1 12 permit. The operator was contacted about the inspection and was present throughout <br />it. The inspection topics below refer primarily to aspects of the 110 operation, but the narrative on page 2 includes several details <br />of the proposed 112 operation. <br />This is a fairly simple operation now, and it will probably continue to be alter the conversion. The pit is excavated to a depth of <br />20 feet or less, by pushing material down from the top with a dozer, then removal with loaders. Most material is crushed then piled <br />by conveyor. Some product is segregated for use in asphalt botching; the material is sold because the botching is not performed <br />in this permit area. <br />The present pit is in the northeast corner of the property on which it is located. Initial expansion will be to the south; later expansion <br />will be to the west. Expansion will proceed toward the concrete irrigation lateral. The land is owner by the operator, even beyond <br />the irrigation lateral, but no excavation will occur within a specified distance from the NE side of the irrigation lateral, and no <br />excavation will occur to the SW at all. <br />Excavation will be to the same depth as the existing pit. Crushing and screening are approved for the 110 permit and proposed in <br />the 112 permit also. Equipment observed during the inspection was compatible with approved activities in the permit, and all <br />structures observed were temporary or portable. Reclamation involves primarily the grading of sideslopes to 3:1 . <br />Along the north edge of the pit floor is a small amount of non-mining related equipment, which is used by the operator in his <br />construction business. This portion of the pit is pre-law and not included in the permit, therefore the materials present do not have <br />to be bonded for. <br />The conversion application materials submitted by the operator (on 3/24/97) were discussed during the inspection. So far the <br />application has not been found to be complete. Without letting this report become a completeness or technical adequacy letter, this <br />inspector will identify a few of the major items noted as lacking and which were discussed while onsite: <br />1 . There must be better representation of the site as it exists now, how it will be mined, and its condition after reclamation. This <br />includes such structures as the overhead powerline inside the east fence, dirt road locations, culverts, fences and gates. Include <br />names of all owners of land (even across the road) and structures (ditches, utility lines). The presumed permit boundary on the map <br />is within 200 feet of many of these features and properties, yet the operator stated that the boundary will be kept 200 feet away <br />from his property line. <br />2. On the application form, the acreage stated is incorrect and must be corrected. If the permit boundary is changed from its <br />present depiction, the acreage will change also. <br />3. The 112 conversion application requires submittal of exhibits, only some of which were part of the original 110 permit. The <br />operator should examine the Division's letter (dated 4/9/97) and the application package, and consult the Rules to properly complete <br />these exhibits. Please contact this office if there are any questions. <br />At the entrance to the pit there was no permit ID sign noted. The boundary markers were observed, though they were not all well <br />installed. The lack of a sign is noted as a problem on page 1, and corrective action explained on page 3. A sign will be required <br />now and after conversion approval. The existing markers (particularly ones fallen down) should be installed more securely.
