<br />Page l36 <br />Construction Materials Rule 6 <br />A list of any mttnicipality(s) within two miles of the prop sed minim <br />operation and address of the general office of each municipality <br />6.4.17 EXHIBIT O - <br />Commissioners and Soil Conservation District <br />Proof that notice, of the permit application was sent to the Bo d of County <br />Commissioners and. if the mining operation is within the bound ies of a Soil <br />Conservation District, to the Board of Supervisors of th local Soil <br />Conservation District, pursuant to Subparagraph 1.6.2(1)(a)(ii). <br />6.4.18 EXHIBIT R - <br />An affidavit or receipt indicating the date on which [he applicatio was placed <br />with [he local County Clerk and Recorder for public review, pursuant to <br />Subparagraph 1.6.2(1)(c). <br />6.4.19 EXHIBIT S -Permanent Man-Made Structures <br />t t 5(a)(e) Where the mining operation will adversely affect the stability of an~significant, <br />valuable or permanent man-made structure located within two hu dred (200) <br />feet of affected land, the applicant may either: <br />(a) provide a notarized agreement between t e applicant <br />and the person(s) having an interest in the structure, that t e applicant <br />is to provide compensation for any damage to the structur ; or <br />(b) where such an agreement can not be r ached, the <br />applicant shall provide an appropriate engineering eva] anon that <br />demonstrates that such structure shall not be damaged activities <br />occurring at the mining operation. <br />6.5 GEOTECHNICAL STABILITY EXHIBIT <br />tt6(al(tl (1) On aaite-specific basis, an Applicant shall be equired to <br />provide a geotechnical evaluation of all geologic hazards tha have the <br />potential to affect any proposed impoundment, slope, embankmen , highwall, <br />or waste pile within the affected area.
