The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted <br />on the previous page should be notified of all corrective actions taken. <br />PROBLEMSlPOSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. Mine identification sign and permit perimeter markers were not observed. Rule 3.1,12111 requires the operator to post a sign <br />at the mine/mill entrance, clearly visible from the access road, which displays the name of the operator, the permit number, and a <br />statement that a reclamation permit for the operation has been issued by the Colorado Mined Land Reclamation Board. Rule <br />3.1.12121 states that the boundaries of the affected area will be marked by monuments or other markers that are clearly visible and <br />adequate to delineate such boundaries. <br />CORRECTIVE ACTION: Posy mine identification sign and permit perimeter markers, as required by 3.1.12. <br />01/22/96 <br />PB or PV PB <br />TOPICISI SM <br />2. Accompanying this inspection report is Notice to Increase the Financial Warranty, totaling 53,071.00. Pursuant to Rule <br />4.2.1121, the operator shall have up to sixty (601 days to post additional financial warranty from the date of written notice of such <br />insufficiency. If the operator disagrees with the Notice to Increase the Financial Warranty, DMG shall schedule the matter for a <br />hearing before the Board. <br />CORRECTIVE ACTION: Pursuant to Rule 4.2.1121, submittal date for additional financial warranty, totaling 53,071.00, is 2/13/96. <br />02/13196 <br />PB or PV PB <br />TOPIC(S) FN <br />3. An unknown chemical, surfactant, is stored and utilized at this operation. The permit states that no chemicals will be stared <br />or utilized with this operation. <br />CORRECTIVE ACTION: Provide Material Safety Data Sheet (MSDSI for the surfactant utilized at this operation. MSDS are available <br />from the product manufacture. Submit a Technical Revision ITRI to the permit application, describing how this chemical is stored <br />and utilized at the mill permit area. TR shall describe how operator will comply with the applicable requirements of Rules 3.1.5110), <br />3.1.51111. 3.1.6 and 3.1.7. <br />01 /30/96 <br />PB or PV PB <br />TOPICISI MP <br />4. Drainage containment structures for the sand tailings stockpile area was not observed. Impervious liner underlying the mill, <br />pond, and tailings stockpile area was not observed. The permit application does not contain a lab analysis of the ore or tailings, <br />verifying that the ore or tailings do not contain toxic or acid forming materials. <br />CORRECTIVE ACTION: Submit a Technical Revision ITRI to the permit application containing lab analysis of the ore or tailings <br />sufficient to characterize the tailings as to their potential to generate "acid mine drainage", as described in Rule 1.1121. <br />01130196 <br />PB or PV PB <br />TOPICISI PW <br />SF, HB, AT <br />
