(Page 3)
HI NE ID f OR PROSPECTING ID f M-77-193
INSPECTION DATE 11/27/96 INSPECTOR'S INITIALS TAS
appear to be adequate to contain the volume of runoff generated from a 30 year - 24 hour
storm event. We therefore direct this matter to the attention of the Wat~ar Quality Control
Division. it seems reasonable and prudent for the operator to install temporary controls or
complete construction of the permanent atormwater Control structures ae soon as possible in
order to properly manage etormwater flows expected during the 1997 runoff season. Currently,
there does not appear to be adequate storage capacity in the existing ponds and there are no
collection ditches or culverts in place to direct and control the flow of stormwater from
pond 2 to pond 3 and from pond 3 to pond 4.
8. The location of the proposed new haul road was inspected. It appeared that a significant
cut and fill may be necessary in order to achieve the proposed design. lA-. Tezak indicated
that all pertinent approvals from the local authorities have been secured and final designs
will be forthcoming shortly. This inspector indicated that stability of the haul road as
well as the upelope embankment may become a permitting issue. The Division will evaluate
this issue following review of the engineering designs.
9. Mr. Don Hellman, mine manager, who was present during previous inspections by the
Division in 1995 and 1996 was no longer with Tezak Heavy Equipment Company, Inc. He will be
missed.
I & E Contact Addreae
cc: Jim Stevens, DMG
Hob Shukle
Water Quality Control Division
4300 Cherry Creek Drive South
Denver, CO BC 222-1530
^ CE
NAHE
Mr. Edward J.
