• (Page 2) <br />MINE ID # OR PROSPECTING ID # <br />INSPECTION DATE 5/17/99 <br />M-99-002 <br />OBSERVATIONS <br /> <br />INSPECTOR'S INITIALS ACS <br />Rule 3.2(7) of the Hard Rock/Metal Mining Rules and Regulations of the Mined Land Reclamation Board <br />specifies that operations applying for a section 112 reclamation permit be inspected after the <br />application is filed and prior to Board consideration. This report documents the Division's <br />observations made during the required pre-operational inspection. <br />The 29-2 well pad was inspected. This is the proposed location for the set of nested wells that will <br />monitor ground water quality upgradient of the first five-year mining panel. The well pad is located <br />in Horse Draw north of the Horse Draw Road. From this location the alignment of the proposed main <br />access road may be observed. The U.S. Bureau of Mines research shaft in Horse Draw was also inspected. <br />The test mine facility ponds were inspected. The ponds have been netted to exclude waterfowl, and are <br />fenced wi[h four-foot high wire fencing topped by four-strand barbwire mounted on wooden posts. Both <br />the fence and the netting were observed to be in good condition. The steep north-northeast facing <br />slope coming down from the facilities pad to the ponds was recently hydroseeded/mulched. The steep <br />slope and other locations around the test mine facilities have been planted with sagebrush as an <br />experiment to evaluate survivability of transplants. <br />Production well 20-2 was inspected. This gave the inspectors a good orientation to the layout and <br />nature of an individual well that may be within a production well field. Pipe racks, piping <br />insulation, and nitrogen feed were inspected. Production well 20-14 was also inspected. The topsoil <br />stockpile associated with this well pad has not yet been seeded. This stockpile must be seeded this <br />fall. The three existing production wells (20-2, 20-3, 20-14) have been developed on a 300 foot <br />spacing. <br />The proposed Piceance Site initial processing facility location was inspected. This is located just <br />off the Yellow Creek jeep trail at the west end of the 5 to 10 year mining panel in an area of <br />relatively dense pinyon/juniper forest. A road has been constructed from the proposed plant facilities <br />site to monitoring wells 20-10 and 20-4 to the southeast. Topsoil was not salvaged from the well pad <br />at 20-4, which is unacceptable under the proposed plan. American Soda is advised that any pattern of <br />topsoil mishandling may result in violations under 34-32-124, C.R.S.
