(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-~~i-~(1~ <br />INSPECTION DATE: ~/i~/n~ INSPECTOR'S INITIALS: ~!? <br />OBSERVATIONS <br />1. This inspection was conducted in response to a complaint of a possible illegal mining operation located <br />at the northwest corner of Weld County Roads 18 and 25. Linda Piper of Fort Lupton, CO submitted the <br />complaint. Present at the inspection were Kate Pickford, DMG and Ted Zadel, Northern Colorado <br />Constructors, Inc. Aggregate Division. Weather at the time of inspection was clear, calm, and <br />approximately 85 degrees. <br />2. The complaint indicates that the neighbors to the site were never aware of notice that the site was <br />being permitted through the state as a mining operation. The complaint further stated that the <br />landowners of the affected land are named Zadel. <br />3. At the time of the inspection, mining activity involving heavy equipment was occurring at the site. There <br />was no sign posted at the entrance to the site. Near the WCR 25 entrance to the site there are a scale <br />and scale house set up. Approximately 4 acres are disturbed at this time in the form of areas stripped of <br />topsoil and stockpiles of material. <br />4. The heavy equipment operator at the site identified himself as Ted Zadel. Mr. Zadel stated that the <br />business manager of the operation is Chris Zadel. Chris Zadel was contact via telephone from the site. <br />Chris Zadel supplied the name of the operation and the state reclamation permit number. The permit <br />number was confirmed through the DMG database. <br />5. Chris Zadel was advised regarding the rule pertaining to posting a sign at the entrance to the site.
