(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~-19R9-O5~ <br />INSPECTION DATE: s_~_ne INSPECTOR=S INITIALS: JD- <br />OBSERVATIONS <br />Completed an inspection of this operation as part of a Mined Land Reclamation Board directive to inspect <br />every mine in the state at least once every four years. The site was last Inspected June, 1999. <br />This is a 9.9-acre Limited Impact permit located about l Yz miles west of Highway 93 just south of 56~h ave. <br />Thisis a smallziayaperation. The operatorcutsaffiayeTSOf zfay fmm nearvertical beds,-stockpiles the material <br />up to the east of the active mining area. This material will be back graded to the toe of the highwall during <br />reclamation. <br />The access road is stable. The lowest staging area is stable. Native vegetation is encroaching into the berm <br />areas and areas that haven't been disturbed for awhile (yucca, sage, rabbitbrush and various grasses). <br />A mine ID sign has been placed at the entrance of the permit area. However, it lists Acme Brick as the operator. <br />The operator/permittee is The Denver Brick Co. The error must be corrected by the corrective action due date <br />(see page 3 of this report). <br />The Financial Warranty requirement was recalculated because it has been several years since the bond was <br />looked at. The revised bond requirement has been estimated at $33,700.
