MINE ID # OR PROSPECTING ID #: rttaan-~aa <br />INSPECTION DATE: a-~tn-m INSPECTOR'S INITIALS: ~- <br />The Division conducted a construction progress of the Phase IV area and monitoring Inspections of the <br />existing facility on 4-30-02. Present during the inspection were Phil Barnes and Terry Mandziac of CC&Y. <br />Inspected the ARD expansion area, geomembrane liner was being deployed due South East of the existing <br />building. This area will be used as storage for Cyanide and other chemicals that will be utilized by the <br />process plant. No major problem noted. <br />Inspected the Arequa Gulch over burden storage area, the storm water management pond for the area was <br />taking shape, according to Phil, it should be completed with in the next few weeks. <br />Inspected the Phase IV LOAD OUT BIN AND CRUSHER AREAS. The geomembrane liner is in place. Drain <br />cover was being deployed at the time of the inspection. Golder QA/QC personnel was over looking drain <br />cover deployment. Work was continuing on both the crusher and Load out bin, no major problem noted. <br />Inspected the Phase II pregnant solution collection area and recorded the following readings from the High <br />Volume and Low volume collection transducers. <br />High Volume Transducer #4 was recorded at 20.5 feet # 5 was at 26.9 feet and # 6 was at 32.3 feet. 9 All <br />measurements are in feet of head. <br />Low volume Solution Collection Transducer # 1 was recorded at .15 feet of head and # 2 was recorded at <br />.21 feet of head. No major problem noted. <br />Inspected the external pond. It was holding about the same amount of water as reported in the last <br />inspection. Readings from the low volume solution collection transducer were at 7.4 and 9.2 inches of <br />hydrostatic head. The certification report for the pond, has not been approved by the Division to allow it to <br />hold process solution. <br />Inspected the Iron Clad Globe Hill areas. Overburden was Bening placed in the area. The new Track Shop IS <br />NOW OPEARTIONAL. Storm water diversion structures for the area appeared to be in good repair, no major <br />problem noted. <br />Inspected the East Cresson mine, it was active at the time of the inspection, no major problem noted. <br />I & E Contat Address <br />NAME: MR.
