:` <br />' • (Page 4) • <br />~ MINE ID # OR PROSPECTING ID # M-81-276 <br />INSPECTION DATE 12/16/99 <br />INSPECTO 2'S INITIALS TAS <br />The following problems (PB) and/or possible Jolations (PV) (and suggested corrective <br />actions) were identified during this inspec~ The problems should be corrected by the <br />dates given, or they will become possible lations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. There was no entrance sign or permit boundary markers <br />posted as required by Rule 3.1.12. <br />CORRECTIVE MEIISQRE3 <br />Post an entrance sign and establish markers identifying <br />the boundaries of the 85 acre permit boundary. Submit <br />photographs on or before February 29, 2000 verifying <br />completion of these tasks. <br />2. There was significantly more than 10 acres of active <br />disturbance as specified in the mine plan. The extent <br />of total disturbance is unknown. Also, the existing <br />financial warranty appears to be very inadequate to <br />complete the reclamation plan for current disturbances. <br />CORRECTIVE ME118IIRES <br />Submit a technical revision on or before February 29, 2000 <br />that allows for more than 10 acres of active disturbance, <br />that includes a revised reclamation cost estimate for all <br />current and/or expected disturbances, and includes a survey <br />of all current disturbances at the Hertzog Pit. The site <br />survey must be prepared by a registered surveyor or qualified <br />professional and must include the following information: <br />1) a map showing the acreage of all current disturbances, <br />2) an overlay or 2nd map showing the extent of current <br />disturbances in relation to the 65 acre permit boundary, <br />3) the location, height, slope, and number of linear feet <br />of existing highwalls, 4) the location and acreage of all <br />slopes graded to 3:1, 5) the location and acreage of all <br />slopes graded to 3:1 and topsoiled, 6) the location and <br />pit floor acreage, 7) the location and volume of all <br />overburden stockpiles, 8) the location and volume of all <br />topsoil stockpiles, 9) the location and acreage of storage/ <br />processing area, and 10) the total acreage of affected land <br />requiring scarification and/or revegetation. <br />February 29, 2000 <br />PB or PV PB <br />TOPIC(S) SM ~, <br />February 29, 2000 <br />PB or PV PB <br />TOPIC(S) MP, FN <br />
