III. COMMENTS -COMPLIANCE <br />Below are comments on the inspection. The comments include discussion of observations made <br />during the inspection. Comments also describe any enforcement actions taken during the inspection <br />and the facts or evidence supporting the enforcement action. <br />Availability of Records <br />Required records were on file and up-to-date through the 2'" Qtr., 2002. The records were checked by Dan <br />Mathews of DMG the afternoon of July l 1. Mitchell Rollings had concluded his inspection prior to the <br />records check. <br />Roads <br />Refuse Haul Road, Conveyor Access Road, and Water Tank Access Road had been previously treated with <br />MgCI, and road surfaces remained stable with essentially no dust generation, despite the dry conditions. <br />Drainage structures associated with the haul road and major access roads were properly maintained. <br />Various riprap lined and rock check haul road ditch outlets had been properly maintained as requested since <br />the previous inspection. <br />The road ditch segment adjacent to "Treasure Island" entrance gate had been better defined as requested, <br />The "sinkhole" feature, located on the light use road between the B Vent Shaft and RS-1 Shaft Site had not <br />yet been filled in, and this work still needs to be done. The hole opening is approximately 3' by 2', and hole <br />depth appeared to be approximately 6' to 8'. It is immediately adjacent to the road. <br />Hvdroloeic Balance <br />Most sediment ponds, and all SAE Sediment traps were dry. None of the ponds were discharging. At the <br />refuse areas, Pond RP-2/3 still contained a shallow pool of water in the west inlet cell; the main cell and <br />southeast inlet cell were both dry. SE inlet cell is largely filled in with sediment, and a track hoe was <br />parked at the site. Operator indicated excavation of the inlet cell would be initiated shortly. There was a <br />shallow pool in Pond RP-4, water level approximately 6' below top of riser. Pond RP-1 was dry, sediment <br />level approximately 6' below top of riser. Pond RP-SA was also dry, sediment level approximately 5' <br />below primary spillway elevation. <br />At the D Portal Area Pond DP-1, water level had been pumped down to approximately 5' below the top of <br />riser, and excavation of sediment from the upper end of the pond had been initiated. A quantity of <br />sediment had already been hauled to the refuse areas for disposal, and additional material that had been <br />pushed up into several piles in the pond vicinity was likewise to be disposed of. Both Slot Storage Ponds <br />were dry, with sediment level in SS-1 approximately 2 feet below top of riser and SS-2 approximately 4' <br />below fop of riser. <br />Processing Pond PP-2 contained a shallow pool of water. A small area of the pond liner on thewest bank <br />was exposed, and protective soil cover will need to be replaced. <br />Upper end of the ditch that runs along the bench to the NW of PP-2 needed to be better defined to ensure <br />sufficient grade. <br />
