(Page 2) <br />MINE ID # OR PROSPECTING ID # <br />INSPECTION DATE: <br />OBSERVATIONS <br />INSPECTOR'S INITIALS: ~.~ <br />Completed apre-operation inspection of this site pursuant to requirements of Rule 3.2(7). <br />The operator submitted an amendment application to increase the size of the permit area from 22 acres to 72 acres. <br />The proposed amended lands mostly surround the existing permit. <br />This is a hilly short grass prairie area. The applicant proposes to mine the sand and gravel by excavating the hills west and <br />southwest of the existing site. <br />A power line extends into the existing permit area about 150 feet, above ground, on the south side of the access road. The <br />line continues underground to a power pole next to an equipment parking area. This structure will be addressed in the <br />adequacy letter (for compliance with Rule 6.4.19). <br />A weigh scale is located along the access road which could be a problem if the structure is not located inside the permit <br />area (assuming this is not a custom scale). Currently, the width of the access road is 30 feet. The road traverses westward <br />into the permit area from Elbert highway. The road appears to be wider than 30 feet where the scale is located. However, if <br />the scale will be located inside the proposed amended area, or the scale facility is not used exclusively for the mining <br />operation, this will not be a problem. But the structure should be shown on the Exhibit maps. This too will be addressed in <br />the adequacy letter. <br />The required notice for the amendment has been placed at the entrance of the permit area. A mine ID sign is posted at the <br />entrance of the permit area. <br />The proposed expanded area has not been delineated. The applicant must adequately mark the amended permit boundaries <br />before disturbing any lands in the expanded area. <br />Some areas of the proposed expanded area will not be disturbed according to the applicant's representative. <br />The operator of the existing operation is stockpiling concrete and asphalt waste in the northeast corner of the permit area. <br />The operator plans to use the material for rip rap at areas within this corner of the county where potential erosion problems <br />are noted. The applicant should include this activity in the permit (importation of concrete and asphalt waste) while this <br />amendment application is being processed. <br />The proposed amended area will include an equipment parking area. Currently, there are several pieces of equipment in the <br />parking area including a diesel tank (with no containment) and several oil drums (again, with no containmenU. Once this <br />area is included in the permit (when the amendment application is approved), the operator must construct containment <br />systems around the diesel tank and the oil drums (sufficient to contain the total volume of diesel or oil, plus 30%). <br />The amendment application will be reviewed in the next week.
