• (Page 2) <br />MINE ID # OR PROSPECTING ID # M-94-117 <br />INSPECTION DATE 10-8-97 INSPECTOR'S INITIALS GC <br />OBSERVATIONS <br />This was a monitoring and compliance inspection to assess the status of Board ordered corrective <br />actions assessed during the September 24, 1997 Board Hearing. Anon-English speaking hand was present <br />for the operator. The weather was clear to partly cloudy. <br />HAZEL-A Adit <br />The He~zel-A adit was inspected. Theze was no visible or audible water inflow at the time of the <br />inspection. The slow leak was still present near the base of the wooden bulkhead and was passing <br />throuch the base of the cinder block wall. The cinder block wall was still about waist high. The <br />measured freeboard behind the wooden bulkhead was 23 inches. A few inches of water was pooled behind <br />the coffer dam, which was operational. The dimensions of the adit were measured in the vicinity of <br />the wcoden and cinder block bulkheads. The adit measures 6' high x 5' wide at the bulkhead. <br />The Hcizel 'A'log book was inspected and was found to be up to date and maintained with detailed <br />entries. <br />Tailia.ge Pond <br />The spray evaporative system was operating. The recirculating pump was in operation and the intake <br />had been set within a cinder block lined sump which had been dug deeper into the fines in the pond <br />bott oni. A screen had been installed over the top of the intake. In a telephone conversation with <br />Dave 7'ip ton, the Division had been told that a double screen had been achieved by setting the pump <br />intake inside a drum with holes. This satisfies Corrective Action No. l.b. in Notice of Violation <br />MV-97-027 issued on September 24, 1997. <br />Use o1: the designation letter "Y" (Y=Inspected and Found in Compliance) in the GENERAL INSPECTION <br />TOPI C:= section of page 1 of this report is not intended to abrogate pending problems and possible <br />violations identified in previous reports filed for this site.
