<br /> <br />Letter to Bob Oswald <br />Date: January 29, 2002 <br />Page 3 of 3 <br />The Annual Fee and Report (due February 2, 2002) for the Topaz Mine, Permit M-1980-055, has <br />been submitted under sepazate cover to the Colorado Division of Minerals & Geology at the <br />Sherman Street address in Denver. Copies of the photos documenting the removal of the oi] <br />barrels and the installation of the new Permit identification sign at the Topaz Mine were also <br />included with the Topaz Mine Annual Report. <br />The Inspection Reports from the December 19, 2001 inspection of the five mines at the Sunday <br />group of mines all have the same Corrective Action item of inadequate, inaccurate, and <br />incomplete maps. In addition, all five Inspection Reports have Corrective Action items for the <br />placement of Permit identification signs (completed) and Permit boundary markers. The <br />Corrective Action dates for these items are as follows: <br /> Updated Maps Boundary Markers <br />Topaz Mine 6/25/2002 3/25/2002 <br />West Sunday Mine 7/25/2002 7/25/2002 <br />St. Jude Mine 6/25/2002 3/25/2002 <br />Carnation Mine 6/25/2002 3/25/2002 <br />Sunday Mine 7/25/2002 7/25/2002 <br />To comply with these Corrective Action items, IUSA will need to locate the correct maps, place <br />boundary markers in the field, and update the maps.
