The following problems (PB) and/o~sible violations (PV) (and suggested cot~ive actions) were identified during this <br />inspection. The problems should be corrected by the dates given, or they will become possible violations. The possible <br />violations should be corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />Correction Date <br />1. Highwall excavation along permit boundaries per the Mining Plan call for 3 :1 slopes with a 30 g / 18 / 01 <br />toot buffer. Mine Plan calls for noise and visual abatement berm to be placed between <br />excavation and permit boundary. Excavation has been conducted right up to permit boundary <br />in southwest corner of Phase 2 and southeast corner of Phase 1. Succession of operations <br />from Skelton to United appears not to have addressed certain made features as recuired by <br />Exhibit S of Rule 6.4.19 <br />A Technical Revision (TR) will be required to address correctinc mining up to permit boundary. <br />reclamation of highwall and other issues listed below. A reclamation bond recalculation will be <br />done to correct for additional costs incurred do to mining that will require moving materials to <br />achieve 3:1 slopes as recuired by the permit. Please submit notarized damage waivers or <br />co~ensation agreement from landowner for fencing along south permit boundary. <br />PB or PV: ~$_ <br />TOPIC(S): ode <br />FS RS <br />2. The ponds observed during the inspection are not correctly located on the Mining Map and 9 /18 / 01 <br />incorrectly documented in permit documentation. Specifically; only one pond is shown on <br />the minino map and documented in existing permit. The location of this pond is shown in the <br />southeast corner of Phase 1. The plan does not address the additional pond in the pit area or <br />the one in Phase 5 that appears to be feeding the in-pit ponds. The Mining Plan states that this <br />Dond is lobe left in place and fed from a well. No well was observed at the pit in the shown I <br />ocation or elsewhere in the permitted area. <br />As Dart of the TR recuested above. submit a new mine map. mining plan and reclamation plan <br />showing all pond locations, pond use, and reclamation. Please supply this office with <br />documentation as to the source of water for the pit. Current documentation shows well permit <br /># AD-13947 as beinc denied and no other water rights secured. A copy of this report will be <br />forwarded to the Water Quality Control District in Montrose for their review of this issue. <br />PB or PV: 38_ <br />TOPIC(S): Jj@ <br />
