The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br /> CORRECT ION <br /> DATE <br />1. There are no markers present at the site, to adequately 05/31/96 <br />delineate the permit boundary. <br /> PB or PV: PB <br />The operator must place permit boundary markers, TOPIC(S): SM <br />pursuant to Rule 3.1.12, to delineate the 5.5-acre <br />permit area. Evidence of the installation of the <br />markers must be submitted to this office by the date <br />shown. <br />2. There is no map of the permit area, or of the area 05/31/96 <br />affected by this mining operation. <br /> PB or PV: PB <br />The operator must prepare and submit a map of the TOPIC(S): AR <br />permit area, showing locatable landmarks, areas of <br />activity, and measurements, by the date shown. <br />3. There is no topsoil salvaged for use in the reclamation 05/31/96 <br />of the site. <br />PB or PV: PB <br />The operator must Clarify the statement in the TOPIC(S): TS <br />reclamation plan to "accumulate" sufficient topsoil, <br />begin salvaging topsoil from all future areas disturbed <br />in this permit, and commit to a plan for obtaining the <br />volumes of soil needed to reclaim all acres disturbed <br />in this mining operation. The operator is not required <br />to begin reclamation, but must submit the requested <br />details of the plan by the date shown. <br />4. The disturbed surfaces in the permit are eroding and 05/31/96 <br />are unstable. <br />PB or PV: PB <br />The operator moat perform any necessary practices on TOPIC(S): SC <br />site to reduce the amount of erosion, control the <br />sedimentation, and reduce the potential for offsite <br />disturbance related to the movement of material. These <br />practices are to carried out in a timely manner, and <br />evidence of performing these activities moat be submitted <br />to the Division by the date shown. <br />
