(Page 2)
MINE ID # OR PROSPECTING ID #: M-1981-276
INSPECTION DATE: 9-14-05 INSPECTOR'S INITIALS GRM
OBSERVATIONS
This inspection was conducted as part of the normal monitoring program established by the Colorado Division of
Minerals and Geology. The inspector was in the area on other Division business and noted the site was due for an
inspection. The operator was not contacted as this inspection occurred purely by chance and convenience.
Signs and permit boundary markers were observed as required by rule. The sign was located at the entrance to the
site. Fence lines, t-posts and day lighted edge to the west, define the permit boundaries.
No noxious weeds were noted along the main ramp road and slopes that appear to be reclaimed. Vegetation along
3:1 slopes include grasses such as slender and crested wheat grass along with some rabbit brush and sage. Cover is
estimated to be around 75% or better.
The inspector noted several stockpiles ofprocessed materials on site. A loader and stacker were parked on site. It is
evident that stockpiles have been worked recently. No major highwalls were noted within the mining azea.
No major erosion or sedimentation issues were observed. Mining debris and materials aze concentrated to a yazd
like area.
The only issue of concern is the lack of proper secondary containment for a fuels and oils. The inspector noted a
500-gallon tank sitting on the ground with no secondary containment. Also observed were numerous 5-gallon oil
containers and 55-gallon drums that were not stored or disposed of correctly. The inspector is noting this as a
problem on the last pace of this reoort with a corrective action and date.
No other issues are noted at this time.
Responses to this inspection report should be directed to Russ Means at the Division of Minerals and Geology, Grand Junction
Field Office, 101 South 3rd Street, Room 301, Grand Junction, Colorado 81501, phone no. 970-241-1117.
I & E Contact Address
NAME: Wes Hertzog
OPERATOR: Hertzog Trucking
STREET: 814 Stout Street
CITY/STATE/ZIP: Craig, CO 81625
cc:
^ CE
^ BL
^ FS
^ HW
^ HMWMD (CH)
^ WOCD (CH)
^ OTHER
