The State was notified <br />regarding my schedule and was able to send someone to meet me to <br />conduct a point inspection. <br />There were no outstanding state enforcement actions on this mine <br />prior to this inspection. <br />Several maintenance items were noted during the initial part of this <br />inspection that required followup at the end of week. One of those, <br />the insurance certificate on file at the mine did not name the <br />operation being covered. Mr. Keegan took note of the problem and <br />immediately called the agent in Los Angeles carrying the policy, <br />CGL770-0516, asking that it be changed. The revised certificate was <br />faxed to the mine and available for review at the end of the week. <br />The other problems noted required on the ground maintenance. A small <br />area around the haul road crossing culvert at Stollsteimer Creek was <br />opened up to remove fill over the pipe. The operator is required by <br />mine plan approval to remove the culvert, in doing so, they cut away <br />some of the fill on the lower end of the pipe. That disturbance was <br />not covered by drainage control. The operator put in filter fabric <br />fence below the area on both sides of the pipe. <br />A third area of concern, were broken contour furrows on the reclaim <br />area. The operator is having trouble with gophers digging through <br />
