ORIGINAL - POBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn_IeRn-sae <br />INSPECTION DATE: I ~/_i 6/nd INSPECTOROS INITIALS: ~I~ <br />OBSERVATIONS <br />The Division conducted a bi- monthly monitoring inspection of the site on <br />12/17/04. The area had received approximately 2 inches of snow the night <br />before. Present during the inspection was Tim Comer of CC&V. Inspected the <br />Phase IV pad area. Very little ore had been added to the Phase IV C area, <br />since the last inspection. According to Tim, the crusher was down for some <br />time after Thanksgiving. The Division sent the approval and acceptance of <br />the engineering certification report for the Phase IVC leach pad area, on <br />November 24, 2004. Solution was being applied to the portion of the pad <br />that was loaded with the first lift of crushed ore. Inspected the Phase I, <br />II, III and IV pregnant solution collection areas and recorded the <br />following readings from the High and Low volume solution collection <br />transducers. <br />Phase I High Volume solution collection transducer # 1 read out meter was <br />not functioning; Tim said the electricians have been notified to <br />recalibrate the meter. # 2 was at 47.6 and # 3 was at 47.9. Piezometer for <br />the pond was at 46.5 feet. Low volume transducer # 1 was at.24 and #2 was <br />at .33. <br />Phase II & III High Volume solution collection transducer #4 was recorded <br />at 35.0 # 5 was at 32.5 and # 6 was at 36.4. Low volume transducer # 1 was <br />at .13 and # 2 was at .61. <br />Phase IV High Volume solution collection transducer # 7 was recorded at <br />33.8 # 8 was at 33.0 # 9 was at 34.6 and # 10 was at 34.8. Low volume <br />solution collection transducer # 1 was at 15.3 and # 2 was at 7.9 inches of <br />hydrostatic head. (All hydrostatic head readings are in feet unless <br />otherwise noted) <br />Inspected the external pond, it was holding a partially frozen water. While <br />in the area recorded the following readings from the low volume solution <br />collection transducers # 1 was at 13.6 and # 2 was at 11.6 inches of hydro <br />static head. <br />Inspected the under drain area, all surface water diversion structures in <br />the area were in good repair. Both the south under drain and flinch pipe <br />were discharging. Inspected the newly constructed storm water diversion <br />structures, which includes rip rapped ditches and ponds, as approved in the <br />East Cresson Expansion Technical Revision in Vindicator Valley. They were <br />found to be in good repair. Inspected the Portland pit area, all the <br />backfilling of the pit as approved in Amendment # 8, is almost completed. <br />No problem noted. Inspected the East Cresson area, it was active. Inspected <br />the Grassy Valley area, where CC&V had applied and obtained to drill a few <br />monitoring wells from the State Engineers Office. The case advancing Drill <br />rig was observed, drilling the wells at the time of the inspection. <br />
