Long <br />Division Director <br />The Division has received a copy of the above mentioned report for the oversight inspection <br />conducted at the Meadows #1 Mine on December 15 and 16, 1992. The report included some <br />references with which the Division found discrepancies. You might want to verify the AFO <br />data base in regard to these items. <br />Item 25, Inspection Frequency, part "a." listed the Date of Iast State Complete Inspection as <br />August 13, 1992. Prior to the December joint inspection, the last complete inspection <br />conducted by the Division was on November 11 and 12, 1992. Part "b:' stated that of the four <br />required four complete inspections over the previous four calendar quarters, the state had <br />conducted one complete inspection. Four complete inspections, one per quarter, were <br />conducted by the Division. Part "c." states that of the required eight paztial inspections, only <br />three were completed. The mine was in temporary cessation one of the last four quarters, so <br />no partial inspections were conducted during that quarter. Over the remaining three quarters, <br />the Division conducted five partial inspections. <br />The fourth paragraph on Page 1 of the inspection narrative states that transformers at the site <br />"are not marked with visible labels...as required by EPA regulation." 40 CFR, Chapter 1, <br />§761.45 requires marking of PCB contaminated or PCB transformers, as defined in §761.3. <br />Transformers not containing any PCB contaminants are not required to be labeled.
