• (Page 2) • <br />MINE ID # OR PROSPECTING ID #: n~_invnaa~ <br />INSPECTION DATE: n~i22/nn INSPECTORS INITIALS: C'I K <br />OBSERVATIONS <br />The followi:zg observations were made during a routine inspection of this 112 site. <br />Signs and boundary markers were in place as required by the rules. This site has <br />never been mined. <br />The west end of the site includes a heavy media sink float plant which was <br />constructed to process mine dump material already present on this site from historic <br />mining. There are no signs of activity, recent or past, in this area. Mrs. Watrous <br />is keeping the permit in the hopes of selling the property and transferring the <br />permit at some point in the future. <br />The three historic mine dumps --- the Slide, Smuggler, and Silver Ore Dumps --- are <br />present on _:ite; none have been disturbed. Brown Gulch empties into Clear Creek below <br />the Slide D,unp, and was running clear and clean at the time of the inspection. No <br />apparent erosion has set in along the gulch. <br />The eastern portion of the permit includes the mining office, the Mendota Mine adit <br />into the hillside, and equipment storage area. The adit was sealed off with metal <br />doors and a powder lock, and the office was kept locked and secure. <br />Rocks blocked off the road to the upper containment pond, but no signs of overflow <br />or erosion <;ould be found in the area. <br />All setback: from property lines, ditches, and creeks have been observed. <br />The Operator was informed that prior to a re-start of mining or processing operations <br />at this site, another site inspection would be required to verify that conditions <br />for operaticn were up to standards, and to check the bond amount for sufficient funds <br />for reclamation of the site. <br />No other problems exist at the site at this time. <br />I & E Contact Address <br />NAME: Mrs. Fabvan Watrous <br />OPERATOR: Jack Pine Mining Co <br />STREET: P.O.
