ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn_tag(i_~ee <br />INSPECTION DATE: 3/t 7anx INSPECTOROS INITIALS: ~- <br />OBSERVATION <br />The Division conducted a water sampling inspection of the site on 3/17/04. The main purpose of the inspection <br />was to collect water samples from the under drain discharge pipes. Present during the inspection were Tim <br />Comer and Lawrence Meyers of CC&V. <br />Collected an 8 oz sample from the 4 inch pipe discharge for Wad Cyanide Analysis ,both the South and North <br />Under drains were dry. Operator collected their own sample .Samples collected were properly iced and <br />delivered to the CDOH&E LAB IN THE AFTERNOON. All the surtace water collection ponds below the under drain <br />discharge points were dry, with the exception of the lower big pond which was holding a small amount of <br />water, which was partially frozen. The Surface water Diversion ponds have been cleaned out in anticipation of <br />spring runoff. <br />While in the area inspected the Phase I, II,II &IV leach pads and collected the following readings from the High <br />and Low Volume Solution Collection Transducers. <br />Phase I High Volume Solution Collection Transducer # 2 was recorded at 23.1 #~ was at 19.9. Low volume <br />solution collection transducer # 1 was recorded at .06 and # 2 was at .55. <br />Phase II and III High Volume Solution Collection Transducer # 4 was recorded at 14.3 # 5 was at 12.5 and # 6 <br />was at 13.2. Low Volume Solution collection transducer # 1 was at .07 and # 2 was at .38. Piezometer for the <br />pond was recorded at 28.7 feet. <br />Phase IV High Volume Solution Collection transducer # 7 was recorded at 1 1.7, # 8 was at 10.8, # 9 was at 22.5 <br />and # 10 was at 12.8. Low Volume solution collection transducer # 1 was at 14 and # 2 was at 7 inches of <br />hydrostatic head. During the night, a power surge blew one of the control panels, which shut off the low <br />volume pumps, and for a brief moment the two feet of head maximum allowable under the permit condition <br />was exceeded. Since the permit condition allows for exceedances under these types of circumstances, a <br />possible problem will not be sited at this time. The operator immediately took the appropriate actions and had <br />the power restored to the pumps and replaced the control panel box.
