<br /> <br />MINE ID # OR PROSPECTING ID # M-BO-244 <br />INSPECTION DATE 8-10-99 <br />(Page 3) <br /> <br />INSPECTOR'S INITIALS BK <br />The Division conducted a water sampling inspection of the Carlton Tunnel Discharge area on <br />8- 10-99. Present during the inspection was, Miss Amy Mekette of CC&V.CC&V had reported to <br />the Department of Health and Environment that they have exceeded the discharge limits for the <br />month of May and the Division wanted to collect its own sample for analysis. The physical <br />inspection began around the phase III area. Backfilling of under ground workings and grading <br />of the Hill was continuing. Drove to the Carlton Tunnel Discharge area which is located <br />approximately 4.5 miles down stream of AG 1.5. and is a permitted NPDES Discharge Point. The <br />three ponds which route discharge from the Tunnel were full and discharging at the time of <br />the inspection. The discharge at the flume read appx. 1832 gpm.This location is known as 002A <br />under the CDOH&E Discharge Permit. Six Bozs sample bottles were used to collect the sample. <br />A split of three Bozs bottles were provided to the operator and accepted. Samples were not <br />preserved on site. After the sampling inspection, a brief closure discussion was held with <br />Mr. Scott Lewis and Mr. Phil Earnes of CC&V.
