• (Page 31
MINE ID rY OR PROSPECTING ID # M-1977-215
INSPECTION DATE 10-19-2001
fall to take advantage of winter and spring moisture.

INSPECTOR'S INITIALS SSS, RCO. ALA
The access road to the lower tailings pond is becoming excessively eroded. Though this road was accepted by the USFS
as a permanent feature, it is still part of Permit M-1977-215 and must be maintained according to the requirements of the
Mined Land Reclamation Act and the Hard Rock Rules and Regulations. Sedimentation from this erosion does not appear
to travel off the affected area or the permit area. However, the erosion and sedimentation has concentrated flows which
could result in future problems in this area. The erosion and sedimentation features do provide disturbed areas for weed
establishment and may have contributed to the significant thistle infestation along the length of this road. Regrading of
this road is required with proper drainage controls to be installed to minimize the possibility of recurrence of the erosion.
Weed control along this road will need to be addressed in any weed control plan to be implemented.
Please note the problem descriptions and associated corrective action dates on the last page of this report. All corrective
actions must be completed by the dates shown. This is to include the written follow-up or photos, describing activities
performed at the site, and/or submittal of required revision materials.
Responses to this inspection report should be directed to Bob Oswald at the Division of Minerals and Geology, Durango
Field Office, 701 Camino del Rio, Room 125, Durango, Colorado 81301, phone number 970-247-5193.
18 E Contact Address cc:
^ CE
NAME Rov Cellan ^ BL
OPERATOR Homestake Mining company ^ FS
STREET P.
