a • <br />J,• .~ • (Page 2) <br />MINE ID ~ OR PROSPECTING ID #` M-99-117 <br />INSPECTION DATE 1-23-96 INSPECTOR'S INITIALS ACS <br />OBSERVATIONS <br />This was a joint inspection conducted with Bob Shuckle and Kathleen Sullivan of the Colorado <br />Department of Public Health and Environment, Water Quality Control Division. <br />The extrusion weld repairs to the tailing pond geomembrane liner completed by COM, Inc. <br />between 12-4-95 and 12-29-96 were found to be unacceptable based on vacuum testing conducted <br />by Colorado Lining Company on 1-16-96. Ae a result of these vacuum test failures, COM, Inc. <br />has engaged Colorado Lining Company to cap or patch all of the repairs done by the COM, Inc. <br />crews Colorado Lining Company will also vacuum teat the welds and patches that they complete, <br />and will certify that the work has been completed and tested in accordance with accepted <br />industry practices and standards. At the time of this inspection, Colorado Lining Company <br />had marked the locations on the liner that require capping or patching, but had not yet begun <br />the repair work. <br />The Hazel-A Adit was inspected. The small pool of water behind the secondary containment <br />cofffar dam in the adit was found to be frozen. As a result, the sump pump used to pump-back <br />leakage from the primary bulkhead was also frozen. This ie assumed to be a relatively rare <br />circumstance, only occurring during periods when the air temperature ie well below freezing <br />for eextended periods. As long as the operator continues to pump water from behind the Hazel- <br />A up to the tailing pond on a weekly basis, no discharge ie likely to occur. If the operator <br />find,a that the sump behind the coffer dam begins to fill with water, and that the pump <br />rema:ine frozen, immediate remedial will be required to prevent a discharge. At this time, <br />a tank heater, available at moat hardware or building stores, should be installed in the <br />sump. <br />The synthetic liner on the upstream face of the primary Hazel-A bulkhead had not been <br />adequately affixed to the ad it rib. The Division recommends that the trailing edge of the <br />synthetic liner be pinned or screwed to the rib, using washers or lath so that the plastic <br />wou1~9 not be pulled past the head of the screw or pin, and that the plastic then be lapped <br />to the rib using gunite or some other suitable and durable sealant.
