• (Page 2) <br />~ MINE ID # OR PROSPECTING ID # M-94-117 <br />/ INSPECTION DATE 8-21-97 INSPECTOR'S INITIALS GC <br />OBSERVATIONS <br />This was a monitoring inspection conducted to monitor the status of the spray evaporation and <br />land application systems and the Hazel-A adit. No one was on-site for the operator. Bob <br />Mason stopped by during the inspection. The weather was partly cloudy but there was no <br />precipitation. <br />Tailings Pond <br />There was approximately 17 inches of freeboard in the tailings pond. This was determined by <br />observing the pond water level on the graduated marker taped to the decant riser. An <br />addit`~onal 6 inches was added to that level to account for the freeboard above the top of the <br />riser. This is the same pond level that was measured in the previous inspection of 7-22-97. <br />graduated marker reading = 11 inches <br />added freeboard amount = 6 inches <br />Total = 17 inches <br />A 3 to 4 inch tear was observed in the liner on the uphill side of the tailings pond near the <br />point where the incoming pvc line form the mill T's in with the sprinkler system. This <br />needs to be repaired. See Problem PB-1. <br />Tailings Pond Spray 8vaporation System <br />The spray evaporative system was not operating during this inspection. This was listed as <br />a violation of the permit in the 7-22-97 inspection and is set for a hearing at the September <br />24-25, 1997 Board meeting. No water was being pumped into the pond. <br />The recirculating pump for the spray evaporative system has not been set into a sump area in <br />the pond. As a result, the water level is just above the foot valve intake making it <br />impos;aible to pump the pond level lower. The operator was supposed to have corrected this <br />in then past and is required to pump the pond until it is dry. This will be discussed during <br />the September Board hearing already scheduled. The foot valve intake of the recirculating <br />pump sits in a 5 gallon bucket with holes drilled in the side. There is no lid on the bucket <br />and it lies on its side. This is an inefficient screen and needs to be improved. The intake <br />must be placed fully enclosed within a screen inside a coarser screen or filter. This has <br />been noted in past inspections and will be discussed during the September Board hearing. <br />The operator's representatives have mentioned in the past that there have been problems with <br />the spray heads plugging up. This can be corrected by placing a shut-off valve at each head <br />so it can be worked on without shutting off the entire system, improving the intake screen <br />on th~a recirculating pump valve and placing an additional filter in the spray line between <br />the recirculating pump and the spray heads. The system must be operating efficiently and as <br />approved. If not, this will be discussed in the September Board hearing. <br />Portions of the spray line along the east downhill side of the pond are not fully on the <br />liner and, when in operation, may not be able to keep all spray inside the lined pond. This <br />will :be discussed during the September Board hearing. <br />HAZBL-A Adit <br />The Hazel-A adit was inspected and the freeboard behind the coffer dam bulkhead was measured <br />at 22." No inflow was observed through the shear zone in the adit roof between the bulkhead <br />and tlae portal, however, the base of the bulkhead was leaking. The primary sump outby of the <br />bulkhead was filled with water but was operating at the time of the inspection. The <br />secondary sump was relatively dry. The leaking bulkhead needs to be repaired. See Problem <br />PH-2. <br />The l,sst entry in the log book in the adit was dated August 4, 1997 and initialed DZ. The <br />operator is required to inspect the adit each day and keep a log. In a followup telephone <br />conve:csation, the operator said that the Hazel `A' has been checked daily and that a complete <br />
