(Page 2) <br />MINE ID # OR PROSPECTING <br />ID #: <br />INSPECTION DATE: 10/09/02 <br />M-1997-098 <br />INSPECTOR'S KAP <br />INITIALS: <br />OBSERVATIONS <br />1. This inspection was conducted as part of the Division of Minerals and Geology's monitoring program. <br />Present at the inspection was Kate Pickford, DMG. Weather at the time of the inspection was clear, calm, and <br />approximately 70 degrees. <br />2. A sign was posted at the entrance to the site. Material was being actively mined and removed from the site <br />at the time of inspection. It could not be determined whether the boundaries were marked, although <br />disturbance on the site appears to be contained in the areas specified on the mine pian map submitted with the <br />original 112 application. Extensive road construction including terracing has occurred at the site, but again the <br />activity appears to fall under the spec cations in the-original 112 permit application: At feast on storm water <br />catchment along the terraces was observed which appears to retain runoff from the face of the slope. No <br />noxious weeds were observed at the site. Runoff at the site appears to be contained within the permit <br />boundaries. <br />3. After leaving the site, a representative of P. C. Management, Inc., Ken Higgs, was intercepted while en route <br />to the site: Mr. Higgs advised that Tim Robertson is not longer the contact for the Flag Creek Quarry site-. New <br />contact information is as follows: <br />Paul Koleman <br />7020 Young Rd., N.W. <br />Olympia, WA 98502 <br />(360)866-8266 <br />4. All activities and completed work at the time of inspection appeared to be following the guidelines set forth in <br />the mining plan for the pit.
