ORIGINAL -PUBLIC FILE
(Page 2)
MINE ID # OR PROSPECTING ID #: nn-1991-nan
INSPECTION DATE: 5/~SrnS INSPECTORpS INITIALS: ~-
OBSERVATION
The Division conducted an inspection of the site at the request of the operator and the landowner who is the
current permitee. When the site was last inspected in January 2005, the operator who was conducting the
operation under the current permitee was supposed to have transferred the permit to his name so the financial
warranty could be adjusted. The Division at that time had requested the operator to place permit boundary
markers rather than natural features as was approved in the current reclamation plan. He was also requested
to provide a new surveyed map of the permit area and do as much recalamtion as possible to lower the
financial warranty liability. Mr. Howard Stone the current permit holder and Mr. Russell Nan Egmond were
present at the time of the inspection on 5.27/05. Two copies of the SO application were provided and briefly
went over the forms. Mr. Egmond the current operator under Mr. Stone's permit had cut the high walls and
conducts as much reclamation as possible. He had also place permit boundary pins and plans to fence the
entire permit area once the sale of the parcel of land which totals over 50 acres that includes the permit area
goes is finalized. The site was not active at the time of the inspection Mr. Egmond also wanted to place small
grovel and other product display area outside of the permit because he plans to import other rock products for
sale from sources other than his permit area. He was informed that as long as he plans to import rock products
for sale from sources other than the Stone Gravel Pit it will be considered as a customer show area and will not
need to be covered by the permit for financial warranty calculation as disturbed area. However, if all the
products were from the existing site it will be considered part of the permit and will have to be with in the
existing affected area of the permit. Mr. Egmond requested that DMG calculate the existing warranty based
on the existing disturbance and approved recalamtion plan for the permit and if at all possible to fax it to his
office by 5/27/05. The needed warranty for the site was calculated and faxed this morning; it is also included
with this inspection report as an attachment. Since the new operator is going to succeed the operation no later
than June 30, 2005, the fact that the site is under bonded will not be sited as a possible problem at this time.
However, if the SO application is not submitted and approved by 7/30/05, the current permitee will have to
post the additional warranty of $14,099 no later than 7/30/05
1 & E Contact Address
NAME: MR HOWRAD STONE
OPERATOR: SAME AS ABOVE
STREET: 8999 TFI f FR Rf~ NC) 1
CITY/STATE/ZIP: FI C)RISSANT Cnl C)RAf)(~ ROR16
CC: RUSSEL VAN EGMOND
( ATTACHMNETS)
Harry Posey (e-mail)
cc: r CE
r BL
r FS
r HW
T HMWMD (CH)
r
r SE
r WOCD (CH)
r OTHER
