1 <br />April 30 and May 1, 1992 <br />Coors Energy Co <br />Box 359 <br />Keenseburg, Co <br />80643 <br />Keenseburg mine <br />Personnel Present During the Inspection: <br />INSPECTION REPORT <br />Robert Gesser, Coors Energy <br />Shawn Smith, Colorado Mined Land Reclamation Division <br />Larry Routten, Colorado Mined Land Reclamation Division <br />Gary Fritz, Office of Surface Mining\Albuquerque Field Office #294 <br />Weather and Ground Conditions during the Inspection: <br />High cloud cover early in the morning, warm and dry <br />GENERAL COMMENTS <br />This was a complete oversight inspection. The State was notified <br />regarding my schedule and was able to send someone with me to conduct a <br />joint inspection. <br />There was no outstanding enforcement action issued by the Colorado Mined <br />Land Reclamation Division pending abatement on the mine prior to this <br />inspection. There were no enforcement actions taken by either the State <br />or myself following this inspection. A number of concerns were raised <br />that were either addressed during the inspection or have been noted in <br />their report will require followup compliance within a specified time <br />period. Those are as follows: <br />1, the certificate of public liability insurance for the Coors Energy <br />Co (policy # 3WM004393-00) issued by American Motorists Insurance that <br />expires 10\20\94 did not have the mine listed as an operation to be <br />covered and the statement regarding cancellation indicated that the <br />insurance agent "will endeavor" to notify the State that the policy has <br />been cancelled. This problem was noted during the review of records. <br />The operator called their agent and had a revised copy with the proper <br />coverage and statement regarding cancellation faxed to the MLRD office <br />by the Monday following our inspection. <br />2. the as built certification for pond #1 could not be located during <br />the record review, Mr. Jesser later found it in the mine plan as Drawing <br />KM 4-5 in appendix M. That information was forwarded to Mr. Routten on <br />the monday following the inspection. <br />3. their NPDES permit for the mine has not been renewed per State <br />approval. There was some question in my mind about what regulation <br />allowed this to be approved. The permitting action was proposed and <br />approved in TR-23.
