MINE ID # OR PROSPECTING ID #: M-~ afro-~aa <br />INSPECTION DATE: a-ia-n7 INSPECTOR'S INrITALS: r;K <br />The Division conducted a monitoring and construction progress inspection of the Phase I , II, III and IV of <br />the leach pad facility and supporting structures on 9-19-02. Present during the inspection were Phil Barnes, <br />Mark Tidquist and Terry Mandziac of CC&V. <br />The inspection began, around the Phase IV 9A) area where the operator thought a haul truck drove over a <br />very small area of the north west section of the certified area, which is still under review. Since the area did <br />not have the required cover thickness for such truck travel, It was assumed that portion of the area was to <br />be removed from the certification report with, a new map submittal. However, when an accurate survey of <br />the double lined and certified area was conducted again after the incident. CC&V and the certifying <br />engineer, confirmed that, the area in question was outside of the double lined area, and there was no need <br />to submit new maps to the certification report for Phase IV (A). <br />Continued the inspection to the Phase IV (B) and (C) areas. Back fill and grading work was continuing. The <br />Phase IV PSSA AREA WAS TAKING SHAPE. According to Terry, the area should be ready for geomembrane <br />liner deployment by next week. No major problem noted. Golder QA/QC personnel were present over <br />looking the work in progress. <br />Continued the inspection the new crusher area. It was not active at the time of the inspection. The old <br />crusher has been dismantled and ready to be moved. No major problem noted. Inspected the main Cresson <br />it area, it was not active. Storm water diversion structures appeared to be in good repair. Inspected the <br />East Cresson area, it was active, at the time of the inspection. <br />Continued the inspection to the external pond area. It was holding some water from recent storms. While in <br />the area, the following readings were recorded from the low volume solution collection transducers. <br />Transducer # 1 was recorded at 18.9 and # 2 was recorded at 16.2 inches of head. <br />Inspected the PARSE II pregnant solution collection area and recorded the following readings from the high <br />and low volume solution collection transducers. <br />High volume solution transducer # 4 was recorded at 16.7 # 2 was at 18.9 and # 3 was at 16.2 <br />Low volume solution collection transducer # 1 was recorded at .15 and #2 was at .13. All readings are in <br />hydrostatic feet of head. <br />Inspected the underdrain area, the pump back structure should be completed with in the next week or so, <br />and that all the underdrain pipes should be in place. All three lower ponds were holding water. The 4-inch <br />and 24 inch south underdrains were discharging. The North underdrain 24-inch pipe was still dry.
