(Page 2)
MINE ID # OR PROSPECTING ID #: M-Rn-iaa
INSPECTION DATE: ~-7r-nn

INSPECTOR'S INITIALS: -B~
The Division conducted a monitoring inspection of the site on 7-21-00. The site is a 110C materials permit
covering 9.95 acres. According to the annual report, and confirmed by the operator's representatives, the
site has been inactive this year. Reclamation has been on going since then. Pit slopes appeared to be at 3:1
and moderately stable. No signs of sever erosion was noted with the exception of a few rills on the west
facing slopes. Since these erosion features face towards the pit, until request for release is submitted, they
do not appear to be a major problem. Storm water protection berm is in place around the perimeter of the
pit and it was in good repair. Topsoil is stockpiled and properly protected. According to the operator's
representatives, if the material tests well for gradation and r-values, they might convert the permit into a
112. If that does not happen, since the resource is exhausted reclamation will have to commence within 60
days after the testing is completed. Permit sign and boundary markers were properly displayed.
I & E Conta Address
NAME: RICHARD T.
