• (Page 2) • <br />MINE ID # OR PROSPECTING ID # M-94-117 <br />INSPEC'TIQN DATE 12/18/98 INSPECTOR'S INITIALS CLK <br />OBSERVATIONS <br />The following observations were made during a follow-up inspection of the site, confirming <br />the completion of several tasks required after the last inspection. <br />1) The following work was completed prior to the inspector's arrival at the site: <br />a) All rocks have been taken off the liner. At this time, the pond has <br />approximately 1'-2' of water in it, and the pipeline from the processing <br />plant has been installed. Double-containment on the pipe has been <br />implemented. The pipeline across the dam for discharge of tailings will <br />be installed when it is needed, with a series of spigots for discharge <br />at different points along the dam crest. <br />b) A 6' chain link security fence sturdy enough to keep out large mammals <br />has been put in to prevent deer and other local animals from accessing <br />the pond. The gates for the fence will be installed today. <br />c) A diversion trench above the pond, to the west, has been installed to <br />prevent runoff from entering the pond. It runs under the fence on the <br />south side, along the western edge of the pond, then exits the area <br />under the fence again along the north side. Hay bales have been <br />installed to prevent erosion on the north side, but not the south side. <br />The pathway under the fence needed to be~cleared on the south side. <br />d) A layer of fill has been placed and compacted on the downstream face of <br />the dam, minimizing erosion to the greatest possible extent at this <br />time. The silt fence below the downstream face has been fixed and is in <br />place at this time. <br />e) The south downstream abutment corner has been graded and backfilled to <br />2H:1V. <br />f) Sumps and pumps have been installed inside the processing plant so as <br />to prevent spills from leaving the building, Only one drain on the lower <br />level still needs to be plugged with cement at this time. When this has <br />been completed, a Technical Revision to remove the door berms may be <br />submitted for approval to DMG. <br />gf The drums of hazardous waste outside the mill have been take off-site <br />and disposed of at a hazardous waste facility. Much of the remaining <br />trash outside the mill has also been disposed of .. <br />h) The south silt pond has been constructed, along with the runoff trench <br />leading from the south side of the site down to it. A runoff trench and <br />drainage pipe from the north side of the mill are in the process of <br />being completed, with drainage from the roof of the mill leaving the <br />site at a point below and south of the tailings pond. <br />i) Piping leading down to the Hazel A adit from the tailings pond has been <br />removed, and this adit should only collect runoff drainage water from <br />now on. This will be verified during the next inspection of the site. <br />Piping still exists to transfer make-up water from the Hazel A to the <br />mill. <br />
