C~ <br />(Page 21 <br />• <br />MINE ID # OR PROSPECTING ID rK M-88-057 <br />INSPECTION DATE 01-11-96 INSPECTOR'S INITIALS SSS <br />OBSERVATIONS <br />This was a routine monitoring inspection of the Canyon Sand and Gravel operation located approximately 5 miles northwest of the <br />Turkey Springs Trading Post on Snow Ranch Road in Archuleta County. The pit was not active at the time of the inspection. <br />All signs were in place and the affected area boundary is delineated entirely by dikes and ditches. <br />Grading of the disturbed areas is taking place as gravel is removed. Reclamation of the pond embankments for the phase I and II <br />portions of the operation is nearly complete. Topsoil placed on the embankments last fall was seeded this spring and the vegetation <br />has established itself very well. <br />Phase IV of the operation has not yet begun but is slated to be the next phase to be mined after one more level of excavation is <br />completed from the bottom of the phase I and II areas. The excavation of phase IV will start at the north end of the pit and progress <br />southward into the phase III area. The final extraction is scheduled to be removal of the southern portion of the phase III area and <br />the northern portion of the phase II area. This change of the mine plan has not yet been revised in the permit. The operator will <br />need to submit an application for a technical revision to modify the mining plan accordingly, before the excavation of phase IV <br />begins. <br />The operator purchased a crusher from a local construction firm and is currently assembling it at the site. The operator required <br />the previous owner of the crusher to steam clean it and replace any hoses or seals that were leaking. These measures were taken <br />to preserve, as much as possible, a clean and contaminant free mine site. The operator is to be commended for his diligent efforts. <br />The financial warranty for this operation was calculated and sent to the operator in January 1995. The operator submitted a <br />financial warranty in June 1995. However, the calculated amount of the financial warranty was 51,761 .00. The financial warranty <br />submitted by the operator was 51,700.00. Canyon Sand and Gravel must submit a new financial warranty certification of deposit <br />form (blank form enclosed) for the total amount of 51,761.00. A notice to this effect was mailed to Canyon Sand and Gravel on <br />August 8, 1995 but to date the correct financial warranty has not been received.
