<br />MINE ID # OR PROSPECTING ID # <br />INSPECTION DATE 5/5/98 <br />(Page 2) <br />M-89-003 <br />INSPECTOR'S INITIALS AJW <br />This was a normal monitoring inspection conducted by Tony Waldron of DMG along with Walt <br />Isenbart of Kit Carson County. The site is located 3 miles west and 6 miles northof Vona, <br />Colorado along the south Fork of the Republican River. Sand and gravel are basically removed <br />from the active channel of the river which is usually dry except for the occasional flow <br />during heavy precipatation events. <br />The site was not active during this inspection but there was some mined material stockpiled. <br />The identifi~;ation sign was posted at the mine entrance but the affected area boundaries were <br />not clearly marked. However, the disturbance appeared to be well within the approved <br />affected area. The operator will be required to install affected area boundary markers at <br />this site and this is identified as a problem on page three along with required corrective <br />actions. There was no topsoil salvaged at this site since mining is occurring in the active <br />channel and reclamation is basically to grade out any remaining material once mining is <br />completed and let future runoff events reshape the channel bottom with alluvial sand <br />deposits. <br />I & E Contact: Address <br />NAME Walt Isenbart <br />OPERATOR Kit Carson County <br />STREET P.O.
