MINE ID # OR PROSPECTING ID #
INSPECTION DATE 5/5/98
(Page 2)
M-89-003
INSPECTOR'S INITIALS AJW
This was a normal monitoring inspection conducted by Tony Waldron of DMG along with Walt
Isenbart of Kit Carson County. The site is located 3 miles west and 6 miles northof Vona,
Colorado along the south Fork of the Republican River. Sand and gravel are basically removed
from the active channel of the river which is usually dry except for the occasional flow
during heavy precipatation events.
The site was not active during this inspection but there was some mined material stockpiled.
The identifi~;ation sign was posted at the mine entrance but the affected area boundaries were
not clearly marked. However, the disturbance appeared to be well within the approved
affected area. The operator will be required to install affected area boundary markers at
this site and this is identified as a problem on page three along with required corrective
actions. There was no topsoil salvaged at this site since mining is occurring in the active
channel and reclamation is basically to grade out any remaining material once mining is
completed and let future runoff events reshape the channel bottom with alluvial sand
deposits.
I & E Contact: Address
NAME Walt Isenbart
OPERATOR Kit Carson County
STREET P.O.
