(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1998-003 <br />INSPECTION DATE 0711212006 INSPECTOR'S INITIALS DEG <br />OBSERVATIONS <br />This inspection of Permit Number M-1998-W3 was conducted by Deb E Gbnima of the DMG as a result of a succession of <br />operator and subsequent bond calculation request made by Steve Baker of 38 Enterprises, LLC County representative Marvin <br />Moore was present during this inspection. The pd ~s located approxmately 70 miles northwest of Craig on County Road No 4, <br />and currently consists of approxmately 15 acres, of which approxmately 13 acres are currently disturbed. <br />Two, clearly-labeled, topsoil stockpiles run the length of the interior perimeter of the eastern and southern permit boundaries <br />Theso stockpiles (combined) contain approximately 21.500 cubic yards of topsoil. These topsoil stockpiles also act as a runoff <br />barrier, diverting water away from the disturbed area. In addition, two small shallrnv (epproximalely 6 inches deep) basins at the <br />north edge of the disturbed area were constnrcted (at the recommendation of the Office of Stale Engineer) to limd offs~te <br />sed~menlation from sigrnticant rain events <br />The ma(only of the site has bean lairly compacted by equipment and is Ilat; the main pit area located at the western end of the <br />permd area and is approxunately 30 feet deep The slopes of the pit area have bean graded and are between 4h,1 v and 3h tv. <br />There was no evidence of erosion on site <br />No reclamation has begun, since the sde remains active However, the site appears to have been maintained well enough so <br />That the necessary P.Iement5 Of successful reclamation are in place. One item of note when reclamation begins, the entire <br />disturbed area should be ripped prior to seeding due to the compacted nature of the silo. <br />Mine entrance sign and permit boundary markers are in place, and topsoil stockpiles are also well•marked.
