(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-~aaa-m3 <br />INSPECTION DATE: Nnv 77,07 INSPECTOR=S INITIALS: r;Rc <br />OBSERVATIONS <br />This was apre-operational inspection of the Hall Irwin Corporation's The Shores site. Jeff Gregg and Jim McFarland of <br />Hall Irwin and Danna Ortiz and Karen Flanders of Rocky Mountain Consultants were present for the inspection. Hall Irwin <br />has applied for an amendment to their existing 112 (c) permit, located approximately 1 mile east of I-25 and 3 miles <br />north of the town of Firestone, Co. The amendment was submitted to change the mining sequence, add additional area <br />to the mining area, and to seal the ponds with perimeter slurry walls. It should be noted that although the area to be <br />mined will be increased, the actual permit area will not be increased or moved. <br />The general mine plan, along with the proposed modifications, was reviewed in the mine office prior to the actual field <br />inspection. Some minor issues were raised during the plan review. The Division is requesting that the following items be <br />addressed: 1) Notify the Rural Ditch Company of the Amendment application: 2) Add an addition existing gas well to <br />the application maps: 3) Label the permit boundary on the application maps. <br />Following the in-office review a field inspection of the site was conducted. The Mine ID sign is posted and the Public <br />Notice was also located. The general mine plan was described, and the proposed new mining boundaries were pointed <br />out in the field.
