The above mentioned site has had one or more problems noted that according to the Division of
Mining, Reclamation and Safety (Division) records have not been resolved to date. According to
our records the following problem(s) was to be resolved on or before 6/1/2006. Listed below are
descriptions and corrective actions for the problem(s). Please make the corrective actions and
provide evidence that the corrective actions aze completed by November 30, 2006. Note if the
required corrective actions are not completed, and evidence provided to the Division by the
required date, the listed problems could become possible violations and would be referred to the
Mined Land Reclamation Board for further action. If the problems have already been corrected,
please provide evidence that that are resolved. Contact me with any questions or concerns

There was not a sign present at the entrance to the mine per Mineral Rules And Regulations,
3.1.12. '
Corrective Action
Post a sign cleazly visible to the road with the required information.
Problem Description
The Sediment Control Structure described in the Operations Plan appears to be only partially
functional (Mineral Rules and Regulations 6.4.7(2)(c)).
Corrective Action
Complete installation of the Sediment Control Structure and maintain As needed.
