MINE ID # OR PROSPECTING ID #: M-i eRn-~aa <br />INSPECTION DATE: a-~~-n~ INSPECTOR~SINITIALS: ~_ <br />The Division conducted a monitoring inspection of the site while in the <br />area to attend CDOH&E'S public meeting regarding the renewal of the surface <br />water discharge permit for Arequa Gulch. <br />Prior, to the site inspection, a meeting with Peter, O'Conner, Scot Lewis and Phil Barnes of CC&V, was held, <br />regarding the recent settlement agreement the company signed with EPA AND CDOH &E. The meeting <br />focused on possible concerns the Division had, with some of the language in the settlement agreement. <br />After the meeting, it was concluded that since the settlement agreement dealt with discharge of non. <br />Processed water under the Clean Water Act, to the waters of the United States. Such being the case, and <br />the fact that the Carlton Tunnel ponds are now classified as treatment ponds under the definition of the <br />Clean Water Act, in the settlement agreement, the idea of removing the Carlton Tunnel Discharge area from <br />Permit # M-1980-244, WAS DISCUSSED. <br />The Physical inspection began near the ARD area. The side skirts were being constructed around the <br />expanded area of the ARD building. No problem noted. <br />Continued the inspection to the Phase IV, leach pad area. Ore was being leached in the certified Phase IV <br />(A) AREA, AS APPROVED BY DMG. Back fill, grading, soil liner fill and geomembrane deployment work was <br />continuing along the Phase IV (A) AND (B) AREAS, The PSSA AREA for Phase IV, was partially deployed <br />with Geomembrane liner, along the West side. Golder QA/ QC personnel were present at the time of the <br />inspection. No major problem noted. <br />Inspected the PSSA areas for Phase I and II, and recorded the following readings from the High and Low <br />Volume Solution Collection Transducers. <br />Phase I Transducer # 1 was recorded at 35.6 feet of head, # 2 was at 38.2 and #3 WAS RECORDED AT <br />37.7 (ALL READINGS ARE IN HYDROSTATIC FEET OF HEAD). Piezometer for the site was recorded at 44.4 <br />feet. <br />Low volume transducer # 1 was at .OS and # 2 was at .21 hydrostatic feet of head. <br />Phase II High Volume Transducer # 4 was recorded at 17.5 #5 was at 12.6 and # 5 was at 16.9 Hydrostatic <br />feet of head. <br />Piezometer for the area was recorded at 28.8 feet. <br />Low volume solution collection transducer <br />Transducer #1 WAS rewrded at .10 and # 2 was at .18 feet of head. <br />From the top of the Phase II toe berm, the under drain discharge area was observed. The pump back <br />system grade was in place, no major problem noted.
