• (Page 3) • <br />MINE ID # OR PROSPECTING ID # M-94-117 <br />INSPECTION DATE 6/17/96 INSPECTOR'S INITIALS ACS <br />Neither of these alternatives is acceptable. COM, Inc. must inspect the Hazel-A adit each <br />day, including weekends, and mu at log the inspections in the log book. Failure to do so will <br />reaul.t in a hearing for possible violation (PV) before the Mined Land Reclamation Board. See <br />item #6 on the last page of this report. <br />The interior of the Hazel-A adit, and the Hazel-A water management facilities were inspected. <br />The sump located outby of the of the coffer-dam was filled to near capacity, but the sump <br />pump had not triggered. The coffer dam had over-topped and water was flowing into the sump <br />outby of the coffer dam, but the coffer dam sump pump had not triggered. I manually <br />triggered both sump pumps and pumped all the water back behind the main bulkhead. It is <br />believed that this action prevented a discharge from the Hazel-A ad it which was imminent. <br />COM, Inc. must reset the float switches on the sump pumps such that the pumps will trigger <br />well below the capacity water surface elevations for the sumps. The float switches must be <br />set .in accordance with pump manufacturers instructions. Using baling wire or other such <br />materials to set the float switch trigger elevation is not acceptable. Also, COM, Inc. is <br />required to inspect the Hazel-A water management facilities at a minimum frequency of once <br />per ciay, including weekends. See item #7 on the last page of this report. <br />The outlet of the pump-back hose connected to the inby sump pump is hanging from the back of <br />the ~sdit in such a way that the hose outlet is below the elevation of the main bulkhead <br />crest:. This situation could result in siphoning of water from behind the main bulkhead, <br />through the pump back hose, and into the sump behind the coffer dam. This circumstance would <br />render the pump back system useless, and could lead to an unpermitted discharge from the <br />Haze:L-A adit. The discharge hose must be rigged such that the outlet is above the elevation <br />of the top of the bulkhead crest. See item #8 on the last page of this report. <br />As part of a compliance action during the summer of 1995, COM, Inc. was required to remove <br />oil Eaoaked soil from the perimeter of the mill building. This soil was placed in 55 gallon <br />drum for transport to a landfill. COM, Inc. was required to provide receipts from the <br />landfill demonstrating that the drama had been properly disposed. To date, these receipts <br />have not been received by the Division. See item #9 on the last page of this report. <br />It wtia clear from observations made during this inspection that the new owners of COM, Inc. <br />are not aware of the various permit requirements for this site. COM, Inc. ie again <br />encouraged to read and understand the reclamation permit requirements. Lack of knowledge of <br />permit requirements is not acceptable as an explanation for non-compliance. It was also <br />clear during this inspection that inspection and maintenance of water management facilities <br />by COM, Inc. is lacking. It is not acceptable for Division inspectors to be operating pumps <br />at t})e site in order to prevent a discharge. Any future releases of process water from the <br />site that are the result of lack of inspection or maintenance by COM, Inc. will be treated <br />as possible violations (PV). It ie also clear from inspection of the liner damage during <br />this report and the severing of the power cable detailed in the 6/3/96 inspection report that <br />this site is prone to vandalism. COM, Inc. is strongly encouraged to put security measures <br />in p:Lace to prevent non-compliance as a result of vandalism. <br />
