Pa e 3 <br />MINE ID # OR INSPECTOR'S <br />PROSPECTING ID #: M-1981-086 INSPECTION DATE: 08/1512006 INITIALS: RMW <br />The following problems (PB) and/or possible violations (PV) and suggested corrective actions were identified <br />during this inspection. The problems should be corrected by the dates given, or they will become possible <br />violations. The possible violations should be corrected by the dates given to reduce their severity when <br />considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be <br />notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS and CORRECTIVE ACTIONS <br />PROBLEM or POSSIBLE VIOLATION: PB CORRECTION DATE: September 23, 2006 <br />TOPICS : SC <br />damage. <br />A section of the perimeter berm has been breached by erosion, creating the potential for off-site <br />CORRECTIVE ACTION NARRATIVE: In accordance with Rule 3.1.6 (3), all surface areas of the affected land shall <br />be stabilized and protected so as to effectively control erosion. Please repair the existing berm where it has been <br />breached by erosion in order to effectively control future erosion. Once the berm has been repaired, submit <br />J <br />PROBLEM or POSSIBLE VIOLATION: PB CORRECTION DATE: September 23, 2006 <br />TOPIC(S): HB <br />DESCRIPTION: Ground water has been exposed in the two dewatering trenches at the north end of the site. <br />CORRECTIVE ACTION NARRATIVE: In accordance with Rule 3.1.6 (1), disturbance to the prevailing hydrologic <br />balance of the affected land shall be minimized both during and after mining by complying with Colorado water laws <br />and regulations governing injury to existing water rights. Please backfill the existing dewatering trenches to a level <br />two feet above the ground water level and, if the operator desires to continue to expose ground water, contact the <br />Office of the State Engineer to determine ifa well permit orwateraugmentaGon plan is required forthe exposure of <br />ground water. Once the trenches have been backfilled to two feet above the ground water level, submit <br />photographs of the backfilled trenches to the Division by the correction date. As an alternative, the operator may <br />obtain a gravel well permit from the Office of the State Engineer allowing him to expose ground water. If the <br />operator chooses this option, the Division will require the operator to submit a revision to the permit to include the <br />
